Shades State Park has some nice camping spots. We also like Jackson-Washington State Forest.

"Wilderness" type: Hoosier National Forest (south of Lake Monroe).
"Improved" type: I'd have to think, about this one, but off the top of my head, I would probably say either McCormick's Creek or Starve Hollow (with Yellowwood in the running too).

Clifty Falls, Turkey Run, Raccoon Lake, Yellowood (in no particular order). It would depend on what is important to you. I think all state campsites are clean and taken care of. Some have better "amenities" than others.

Oh, and I should add I rarely (if ever?) do commercial campgrounds and have never done one in Indiana. So . . . .
